Description:
Use this code to add a 3-part date field to your form. It has a
few bells and whistles. First, it will only allow the user to type
in numbers. Second, it will automatically tab from field to field
as the data is entered. Lastly, it comes pre-filled with a value
that will disappear as soon as the user clicks on the field to enter
